If you have purchased a qualifying MotorGuide product fill out the form and mail it in with the UPC code, according to the instructions provided.

The MotorGuide rebate offer is valid with the purchase of eligible MotorGuide products. Offer applies to purchases between February 15 and April 30, 2012 from participating dealers and retailers. Offer valid only in the continental US - void where taxed, prohibited or restricted by law. Rebate redemption materials must be postmarked by May 7, 2012. Only one redemption allowed per product / per family, household or address. Offer is not transferable. Offer is not valid with any other offer from MotorGuide or affiliated companies. Offer not valid where prohibited or taxed. Offer is subject to change or elimination without notice. The offer is only available to end consumer - no claims from retailers, dealers or distributors are eligible. All submissions become the property of MotorGuide and will not be returned - please keep copies for your records. No responsibility will be assumed by MotorGuide for lost, incomplete or illegible requests or misdirected mail. To be eligible, consumers will mail in the completed warranty registration card or rebate form, a copy of the sales receipt, and the UPC code cut from the packaging. Please allow eight weeks for rebate check to arrive.
